AI summary of the Reddit excerpts below — not a quote

Isotretinoin is widely regarded by users as a highly effective, often permanent solution for stubborn acne, though it is frequently viewed as a "last resort" due to its significant side-effect profile. While many report successful long-term results and the prevention of relapses, the treatment is associated with systemic discomfort and requires a multi-month commitment.

Pros

  • Highly effective for clearing acne permanently in many cases
  • Inhibits long-term relapse of breakouts according to user-cited research
  • Effectively targets oil production to treat stubborn or hormonal acne

Cons

  • Systemic side effects including muscle aches, pains, and weakness
  • Increased sun sensitivity that can cause significant facial redness
  • Potential concerns regarding hair loss, particularly for those with existing thinning

Caveats

  • Often described as a treatment of last resort rather than a starting point
  • Initial weeks can cause physical symptoms similar to a hangover or a cold
  • May be prescribed alongside other medications like Spironolactone for stubborn cases

Best for

  • Individuals with persistent acne that has failed to respond to other treatments
  • Those seeking a long-term or permanent solution to acne breakouts
  • Users willing to tolerate a 6-7 month course of treatment

Avoid if

  • You are looking for a mild, first-line topical treatment
  • You have a high concern for hair loss or thinning (AGA)
  • You cannot manage periods of muscle fatigue or extreme sun sensitivity

Even on the low dose regime you will likely have muscle aches and pains and maybe some muscle weakness (in the first few weeks). It can feel like you have had a hard day at the gym along with a hangover or cold.

I suffered from acne for at least a decade before I started tret, it took at least 3 months to see actual improvement and the total course for me was 7 months.

I'm really concerned about the sides with isotretinoin, particularly hair loss. I already have AGA and don't have many hair to begin with.....losing even more would be a nightmare.

I'm also concerned if the dermatologist was one of those who puts everyone female with acne on isotretinoin and Spironolactone. ... I've always thought of isotretinoin as the last resort treatment for acne?

I have been using tretinoin since the last 3 months and combined it with 10mg isotretinoin the 2nd month onwards. I have been tolerating it pretty well i would say except when I am out in the sun my face get red.
