I do know that Nordstrom has a reasonable selection of higher-end shoes in women's size 4 (US), which is indeed not easy to find.
Zappos and Nordstrom are two sites that will stock nicer shoes in size 12.

I’d go to Nordstrom, go to the dresses, ask for help immediately, don’t wander and get frustrated. The staff there are almost always amazing. They can do alterations to help fit properly in the chest. It’ll be worth it.
Try Nordstrom. They have white belts of different widths, and maybe you'll find what you're looking for.
