Porto:The riverside area is beautiful. Do a port wine tour if you're into that - there are tons of cellars and it's actually really interesting. There's also an amazing old bookstore if you want to see something different.
And absolutely do include Porto even if just for 2 days. I prefered it to Lisbon
For what it's worth, I remember Porto being a bit easier than Lisbon, accessibility-wise.

Portugal. They are culturally different enough it will be a totally different experience from Spain, but close enough you won’t waste much time in transit between the two. I love the Algarve region, but Lisbon is great for a first visit and you could also visit Sintra/Cascais as a day trip if you wanted. Porto is also beautiful, but I think for a first trip I’d recommend Lisbon over Porto.
I'm going to throw my weight down behind Porto just because you're going with your wife. Beautiful romantic views and walks (the walk on the upper level of the Luis Bridge at night is one of the most stunning in the world), easy bus to the beach, great pudims and egg tarts, lots of wine, and I felt like we had a good sense of the place in 3 days (plus was able to sneak in a day trip by train to Braga).
We had minimal prep for our trip to Portugal; we only booked our Porto accoms just before we left Lisbon. Such a great place to get around. Nothing was too hard and the people were awesome.
I’ve got a trip planned to Porto at the end of the year (location may change, it’s all a bit up in the air lol)
