Just across a bridge to the west is Isla San Juan where "Old San Juan" is. This too is very popular with tourists (this is where the cruise ships park), but is worth spending an afternoon walking around. It's where all the pictures of colorful buildings and the white "castle" are taken and it's very pretty.

Never been to St Croix, but I will warn that Old Town San Juan is basically on a hill. It will NOT be ADA friendly for your friend.

Photo one was in Ponce, 2-4 were on the beach in Condado in San Juan, photo 5 was in Rincon (Playa Los Almendros), 6 and 7 were in the rainforest of El Yunque, 8-16 were in Old San Juan, 17 and 18 were in Ponce, and 19 was at the night market in Rincon. All amazing places- highly recommend!
